Fair Mortgage Lending Act Forms
The Fair Mortgage Lending Act, Act 554 of 2003, (the “Act”), replaces the existing Arkansas Mortgage Loan Company and Loan Broker Act, Act 806 of 1977, as amended. The Act encompasses sweeping changes in the licensing requirements for those persons conducting mortgage lending activities in Arkansas.

Instructions for accessing and completion of interactive forms:
WARNING: If you use the interactive form without following the steps
below, the form will not be saved.
- Move your cursor to the form you wish to use.
- Right click.
- In the pop-up list, select “Save Target As.”
- A “Save As” box will appear.
- Choose where you would like to save and retrieve the file. For
convenience you can select the desktop – it will appear as an icon
on your screen. Either way, a “Download” box will pop-up and ask
if you want to open the file.
- Choose “Open.”
- The form is mostly interactive in all the areas that
require information. Place your cursor on the areas requiring information,
and begin typing.
- Save your file when completed, by selecting “File” on the menu
bar, “Save As,” then the file name will appear in the “Save As”
pop-up box, click “Save."
- It will ask if you want to replace the existing file, click
“Yes.”
- Print your form and finish by completing with signatures and
notarizing, if applicable.

To apply for an initial license, an Applicant must submit the following to the Arkansas Securities Department (“ASD”):

Licensees are required to keep all information on file with ASD current. If the information contained in any document filed with ASD is or becomes inaccurate or incomplete in any material respect, the licensee shall file FMLA Form 011, Notice of Changes in Licensee Data and/or FMLA Form 012, Notice of Changes in Branch Data correcting the information contained in the document within thirty (30) days from the date on which the change takes place. Any licensee that does not comply with this requirement shall pay a fine equal to Ten Dollars ($10.00) for each day the licensee fails to file a correcting document, up to a maximum of Six Hundred Dollars ($600.00).
